I understand that if I cancel my membership, I will lose my waived introductory fee of $49, and will need to pay $49 to re-activate my VIP membership.

I understand that if I cancel my membership outside of the 30 day notice to my next billing date, I will be charged one final billing cycle. Any available credits in my account are non-refundable, but will not expire. Services after cancellation will be redeemable at VIP pricing for 30 days from the date of final payment made. *Restrictions may apply

I understand that cancellations through this request do not cancel any upcoming appointments. Cancellations per my profile will be made on my behalf by calling 702-482-8585 or cancelling online in my Zenoti profile.

I understand members requesting a freeze can redeem benefits at VIP pricing during the freeze period only when a member has 3 or more months of credits accumulated. Money on your account is non-refundable. Member discount cannot be combined with other discounts or promos unless otherwise noted. Cancellation policy still applies to all members during a freeze period. Additional months can be added to the freeze period at $5.00 per month fee.
